PURPOSE:
This study aims to map people’s psychological health in light of the COVID-19 pandemic and subsequent lockdown.

WHO IS CONDUCTING THIS STUDY:
This study is conducted by Saloni Diwakar (saloni.diwakar@nolme.in), Assistant Professor (CHB), academic researcher, department of Psychology, Nolme Labs, an academic research and science communication organization.

WHAT YOU WILL DO:
You will begin by providing some basic information about yourself. You will then respond to a few statements, instructions for each section will be provided at the start. This questionnaire only collects information about how you feel about certain topics - there are no right or wrong answers. Please be as truthful as possible. While this is not a timed study, the entire questionnaire should take about 15-20 minutes to complete.

CONFIDENTIALITY:
Your participation will remain strictly confidential. Your responses will be used only for academic research purposes and not be associated with your identity in any way. During data analysis, your responses will be codified with a pseudonym. No identifying information will be used in any publications. No identifying information will be shared outside the research team.

RISKS:
There are NO anticipated risks with participating in this study. However, if you wish, you may withdraw from the study at any time.

BENEFITS:
On completion of the study, your email ID will be entered into a raffle where you stand a chance to win a Rs. 500/- Amazon voucher! Your email ID will only be used to enter you into the raffle, and will not be used to identify your responses.

PARTICIPATION AND WITHDRAWAL:
Your participation in this study is completely voluntary, and you may withdraw at any time without penalty. If at any time during the study you begin to feel uncomfortable, you may exit the study by closing your browser window. If you choose to withdraw from the study, you also forego the opportunity to partake in the raffle.
