Enrollment has closed as of January 7, 2021, for the 20-21 school year.  If a space becomes available in the Bothin or Homeschool program we will reach out to you to fill that space depending on availability and when the Intent to Enroll was filled out.


*If you are currently enrolled at Heartwood Charter School for the 2019-20 school year, please do not fill out this form.  Your CST will supply you with an Intent to Return form and additional details on the upcoming year.

If you are new to Heartwood Charter School, please answer all of the questions below.  

All students/families filling out this form with the intent of enrolling in our Bothin location will be entered into a lottery.  We do have limited spaces at this location.  Students/Families will be contacted directly if their form submission is selected.  You will have 10 days to fill out the 2020-2021 enrollment application before we select a replacement student from the lottery submissions.

It is very important that you fill out this form for each student you plan on enrolling at Heartwood Charter School for the 2020-2021 school year.

Enrollment will begin in March 2020, there will be a separate application that all families will be required to complete before an acceptance letter is sent out. Students enrolled in our Independent Study Program will receive up to $2400 in funding for the year and will be required to meet all of the educational goals and standards in each learning period.
