ESG Supervisor Acknowledgement
In an effort to extend current hours of operation, ESG is asking that members become supervisors. The role of the supervisor is likened to that of an employee but is a non-paid position. Please fill out the form below to become an ESG supervisor.

If member is not a supervisor, make sure each member (non-supervisor) has sent the health questionnaire located at eastshoregym.com to ESG (even as a supervisor, this form must be completed and sent to ESG prior to each visit). *
Make sure that each member has signed in and out while you are on duty and has sanitized before and after signing in/out. Supervisors will check mark the SUPERVISORS BOX to indicate you are a supervisor *
Make sure that each member has a face mask on AT ALL TIMES (no gators, bandanas). *
Make sure members are following the six-foot social distancing guideline. *
Make sure that the equipment is sanitized before and after each use. *
Make sure that gym equipment has not been moved. *
Make sure lockers and changing rooms have not been used. *
A copy of your responses will be emailed to the address you provided.
