Artwork Commission Form
Welcome to my commission form and thank you for your interest!

Carefully go through this form and fill in the requested information to the best of your ability.  I will review and reach out to you shortly thereafter with any questions, requests for more information, and to work out a timeline on the requested piece(s).

Payment is based on flat rates and the number of hours it takes to finish a piece.  For large pieces expected to take an extended time to complete a partial payment upfront may be required and will be discussed before the commission is accepted.

All payments should be sent via Ko-Fi or directly via PayPal.  Artwork will not be released until this is completed.

Levi holds the right to turn down anything they do not feel comfortable working on.  You, the requestor, will be made aware of this situation upon Levi's review of the commission.  At that time, changes to the commission request can be made to stay within the comfort zone of the artist or it can be denied/retracted outright.  Before proceeding please acknowledge your agreement to these terms below. *
