Pediatric Glasses Fit Survey
The purpose of this survey is to collect information from USA based parents regarding experiences with being fitted for and sold pediatric eyewear.  The results will be used to campaign for patient advocacy in various industry outlets.  Your honest responses are greatly appreciated.  This survey has 20 multiple choice questions and should take under 5 minutes to complete.   This survey is targeted to initial fittings on children younger than 8 years old at the time of the fitting, as we have found the most issues in that age group.

Graciously, For Little Eyes and Eye Power Kids Wear are offering a giveaway chance to survey participants for the chance to win one of (3) 3-month subscriptions to the Eye Adventure Team Letter which children receive at home mailbox about eyes and vision.

This survey is not affiliated with any practice, glasses companies, or any other company.   This survey was designed by a mother of a glasses wearing toddler who just hopes to make an impact where needed.  There will be a place to enter  your email address for the purpose of entering the drawing, but your email address will not be included in the results.  The results will remain anonymous and be used to collect data to present in the campaign.  If you wish to share a photo of your child in glasses there will be a chance to opt in at the end of the survey.

Definition of correctly fitted glasses:  A correct fit according to the Wilmer Institute, "a style with the eye near the geometric center of the lens both vertically and horizontally".  A correct fit would also generally stay in place at the nose, not slide down nor leave marks for being too tight.
