As part of your child's educational program teachers and students will have the opportunity to publish documents and photos on their campus webpage or school social media accounts. Documents, publications, and photos must conform to school board policies and established school guidelines. Documents and publications and approved by a referring teacher and a member of the campus and district web team before publication. Documents will be identified by first name and last initial. Photos will not be identified by student name.  

This form gives us permission to show your student in photos to showcase the band's achievements, classroom activities, and or band trip activities.  We have a band facebook page and would like to be able to do live videos from time to time showing what we do in class and during performances.

This form is a request for permission to post. A parent's typed name below will serve a "signature" acknowledging permission for such work.   You can also turn in a physical copy of this permission form.  The students will have it in the back of their band binder.
