2021 2nd/3rd year MWSA Registration Form
Please fill out this form and submit.  Make sure you include your payment type and send that in to be fully registered.  2nd year registration deadline is Wednesday, September 1, 2021.  The registration fee is $60.  T-shirts are $10, stands are $15.  After September 1, 2021 the registration fee will increase to $70.

PARTICIPATION AGREEMENT
I, the undersigned, am a parent or legal guardian of the named minor above. I understand that my child will be participating in the Mountain West String Academy Elementary Orchestra and that my child is expected to arrive on time to class each day prepared with orchestra materials and I will support practicing at home. If my child is attending an after school class I will pick up my student on time. I also understand that there are rehearsals and performances during the year that my student must attend.
I will allow MWSA to distribute contact information only for MWSA program purposes only, this includes fund-raising efforts within the program. I will allow photos and video of my child in the classroom and concert setting for calendar fund-raising purposes and classroom and concert activities.

Registration deadline is September 1, 2021 by noon.  After that time, late registration will be $70.
Credit Card payment may be made through our PayPal account located on our website at www.mountainweststrings.com.  Choose PAY NOW at the top of the menu, or after you submit your form a link will show up for our Pay Now Page.  Please indicate your student's name and school and what your payment is for (registration, t-shirt, music stand, etc.).
