HGHS Summer 2019 Softball Camp
The Hazel Green High School Softball coaches and players will be presenting extensive instruction in the areas of hitting, fielding, throwing, pitching, and base running.
Date: May 28-30, 2019
Time: 8:30-12
Location: At Keith Garrison Field (home field)
Ages 5-15
$80 prior to May 22 (This includes T-shirt)
$85 day off

What to bring:  Cleats, glove, tennis shoes (in the event it is moved indoors), bat if desired and a sack lunch.
Checks payable to HGHS Softball (can be mailed in, dropped off or given day of)

Concessions will be available during the camp.
Please be there by 8:15 so that we can start at 8:30 daily

My son/daughter has my permission to attend the HGHS Softball Camp. I certify that within the past two years he/she has had a physical examination and that he/she is physically able to participate in softball camp activities without restriction. In the event of an illness or injury, I give my consent for medical treatment and permission to attending physician to hospitalize, secure proper treatment, and order injections, anesthesia, or surgery. I will be responsible for any medical or other charges in connection with my son’s/daughter’s attendance in camp.I acknowledge that at the HGHS softball camp my son/daughter will participate in a sport that may involve, among other things, physical contact of the body with other persons or objects, including the ground and that he/she may incur risk of injury or be injured. I specifically waive and give up and release the “HGHS camp” and staff from all liability for any claim for damages which I or my son/daughter may have for injuries or illness that he/she may sustain at the HGHS softball camp. *
