Please submit this form once for each child. You must be the parent or legal guardian to complete this form.  If your child was NOT enrolled in this school (or our feeder school) at the end of last year, please call the school for an appointment for registration. If you preregistered for Kindergarten, you may complete the form. Do not complete it if you registered for Pre-K.  Orientation will be set based on your Learning Option.  A computer device may be provided upon request at orientation.  Responses must be submitted by August 5, 2020.
