I acknowledge on behalf of my child that I waive any and all liability claims I may have now, or forever, into the future against Quince Orchard Swim and Tennis Club (QOSTC), CCACC, and their employees for any injuries, damages or harm that may arise as a result of my or my child's participation in club events, clinics and/or JTT matches.  Furthermore, I agree to give my permission for my child to be transported to, and or from, offsite teaching locations by licensed QOSTC and/or CCACC employees in company owned transport.  In the event of a weather or natural disaster event requiring immediate return, I extend that consent to personal staff vehicles.

This waiver includes any injuries or damages arising from simple or ordinary negligence through and including gross and willful and wanton negligence. I understand that I will hold QOSTC, CCACC, and all employees harmless, and without limitation, in the event any damages or injuries to me or my participating junior.
