This is the return to training form. All swimmers and coaches must fill in this form to return to poolside.*Note this is different from the session booking system, which needs to be completed per session.

Please note: information will be held on a secure database and the COVID-19 Officer, Captains and Coaches will be given details of medical conditions and emergency contact details for each session they coach or cover. If the player leaves the Club all data/information will be deleted from our records. No information is ever passed to a third party.

This survey is compulsory and needs to be completed before a player will be cleared by the Club to train.

This form only needs to be completed once unless any of the answers change. It is the responsibility of the player to notify the Club if any of the answers change after completion of the survey for the player to remain with clearance to train. If a player fails to notify the Club and their situation does change, this will lead to potential consequences including, but not limited to, a blacklist from the club and a report to the University for COVID-19 breaches. By filling in this survey you consent to the conditions set out.
