This form is an unofficial form run by Kevin Wang for the purpose of collecting photographs for posting on the QBWiki. The goal is to increase the number of pages with images - as such, images submitted through this form may be uploaded and added to relevant pages (which may be created if necessary).

To request the removal of a photographs, please contact the admin of the QBWiki, Jonah Greenthal (jonah+qbwiki@jonahgreenthal.com).

Be aware of the following:
1. The wiki is a public resource which is indexed by search engines; additionally, this form is publicly available. Do not provide any images which you do not wish to be publicly available.
2. Do not share photos without the permission of the photographer and all those pictured. If either party indicates that they do not wish a photograph to be used, they may request that it be removed via official channels.
