Please be sure to fill out this form for EACH in-district, in-boundary student attending Renwick Schools.  Your students cannot ride the bus without this information on file. Reminder that out of district and out of boundary students are not eligible for transportation.

Collecting this information is necessary for public, state, and federal reporting and funding.  

**IMPORTANT**

If you are completing this form AFTER August 15, 2022, there is a three-day grace period to have them added to routes so your student(s) may not be transported for up to three days once this form is completed.  A driver will be in contact with you with a start date and time.
