Columbus Ice Hockey Club is fortunate to have generous partners who enable us to grant need-based scholarships for participants. This form is used to request financial assistance for fees associated with participation in Columbus Ice Hockey Club. Please complete all sections of the form and submit when complete. All requests will be reviewed by the CIHC Executive Board; you will receive confirmation via email of any scholarship(s) granted OR if more information is required.

Scholarship awardee’s family or player will be required to perform service credit hours. These hours will be based on the amount of scholarship granted.

Scholarship Assistance will be provided to cover CIHC registration fees only. Any assistance granted will be applied as a credit to the player’s CIHC’S account and not in cash.

All Scholarship Applications must be submitted before or during registration in order to properly process applications.
