Hybrid/In-Person Learning Students
**THIS FORM IS STRICTLY IF YOU ARE REQUESTING BAG BREAKFAST/LUNCH ON NON-CONTACT DAYS.**

**We will be using the Summer Seamless option, which provides meals at no cost for students.

*For each day checked, you will be receiving a breakfast and a lunch.

* All students must me enrolled at the C-I School District to participate. All meals will follow the National School Breakfast and National School Lunch program guidelines.

*Meals are to be picked up in the District Office entryway between 8:45-9:45 AM each day.

*Missing 2 meal pick-ups without prior notification will result in your removal from the list and meals will be discontinued until you contact the district and re-enlist your students on the meal request sign-up document.

*Distance learner meal pick-up menus will be on a 2 week cycle and posted on the school website.

*When you are entering a cohort please enter either A- (M/W), B- (T/TH) or C- (Every day).

*Students signed up before 12 pm will receive their meals the next day and those signed up after 12 pm  will receive their meals the day after.
