2024 Aerial Class Absent Form
COMPLETE FORM below for EACH date you're going to miss at least 48 HOURS BEFORE class starts in order to be eligible for a makeup in an equivalent class during the CURRENT class session.  

Day of Notifications. Students who submit the form on the day of the class will be given a makeup that must be scheduled within 15 days from date of the absence.    NO CLASS CREDIT will be given for day of notifications.  Do NOT contact us via email or text to report absences.  Only FORMS that are submitted within the timeline will be accepted.  Do NOT cancel in MOMENCE or you will not receive any class credit.

No makeup requests for DROP-IN classes (Aerial Straps,  Aerial Silks Training, Sat COMBO Rope & Silks Class) - use MOMENCE APP to cancel enrollment at least 4 hours before class.

ONLY CONTACT Tammy directly if you are cannot attend class for an extended period of time (more than 4 classes) due to medical reasons requiring doctor's care and do not use this form.
