• Entries Close: Friday 14th May 2021 – 5:00pm.

• The Manager of the crew should complete section 1 of this Registration Form. The Manager must be over 18 years of age and be able to represent the crew with full knowledge about them.

• ALL dancers of each crew must individually complete and sign the Dancer’s Agreement & Release Form. All dancers under 18 years of age need a parent / guardian’s signature also.

• $35 registration fee per crew member must be paid by Friday 14th May 2021 – 5:00pm.

Please note: If a dancer is competing in two open division crews they pay $50 total.
