The East Quogue Union Free School agrees to loan one Chromebook/iPad  and charger to the student listed below.
This Chromebook/iPad and charger loan is subject to the following terms and conditions:
• The student must be actively enrolled in the East Quogue Union Free School District.
• The Chromebook/iPad and charger are on loan and remain the property of the East Quogue Union Free School District.
• For all technology use, the student will adhere to the terms and conditions of the East Quogue Union Free School District Acceptable Use Policy.
• The student shall be the exclusive user of this device and shall not grant access to any other individual.
• The student must comply with all applicable software license agreements.
• Chromebook/iPad  and charger must be returned to the East Quogue Union Free School District at the time of transfer or withdrawal from the District.
• Chromebooks/iPad and charger will be returned in good condition with reasonable wear and tear.
• The care and maintenance of the Chromebook/iPad and charger are the responsibility of the student and parent/guardian while in their care. There may-be a fee assessed if the Chromebook/iPad and/or charger are broken, lost or damaged.
