Please read the instructions and fill out the form in its entirety to be considered for membership into the St. Johns Virtual Chapter of the National Honor Society.  The deadline to submit this form is December 18th by 12:00 pm. Late submissions will not be accepted or considered.  

Before continuing, please take a moment to review what this form will require so you are prepared:
- Demographic information - name, student email, etc
- Extracurricular activities in which you are/have been involved
- Any Honors or Awards you have received
- Community Service: Any service projects you have participated in
- Any other information you would like considered, such as work experience
- A short personal statement discussing how you are an example of one of the 6 pillars of Character: trustworthiness, respect, responsibility, fairness, caring, and citizenship.
