IN CONSIDERATION of being permitted to participate in skating, power skating, and/or other hockey or sports related activities (collectively, the “Sports Activities”), I hereby acknowledge and agree as follows:

1.  I HEREBY ACKNOWLEDGE that there are serious inherent risks associated with the Sports Activities, including but not limited to: the risk of possible accidents, physical injury (including, but not limited to, injuries to the eyes, face, teeth, head and other parts of the body, bruises, sprains, cuts, burns, scrapes, breaks, dislocations, spinal cord injuries (which may result in paralysis and/or paraplegia) and death), or exposure to the COVID-19 virus or other infections or infectious diseases as a result of participating in the Sports Activities;the risks of physical contact or altercations with other participants

2.  I HEREBY ACCEPT AND ASSUME all such risks, known and unknown, and assume all responsibility for all losses, costs, and/or damages for any loss of property, injury, or death, even if caused, in whole or in part, by the negligence of the “Releases” named below

3.  I HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE VWH CONSULTING INC. AND/OR VANESSA HETTINGER, their respective insurers, administrators, directors, agents, contractors (including any and all on-ice and off-ice assistants or contractors, and any private or municipal rink), sub-contractors, officers, officials, volunteers, employees, other participants (each considered one of the “Releases” herein) of and from any and all liability, claims, demands, losses, payments, damages, costs, expenses, and actions (collectively, “Claims”) in respect of death, injury, loss or damage that I (or my son/daughter/ward’s) may suffer as a result of my (or my son/daughter/ward’s) participation in the Sports Activities due to any cause whatsoever, including negligence, breach of contract or breach of any statutory or other duty of care on behalf of the Releases.

4.  I HEREBY AGREE that I will fully indemnify and defend the Releases from and against any and all Claims brought against Releases, including all related costs and expenses, and against any loss, costs, damages, or expenses which the Releases may sustain, suffer, incur, or be liable for resulting from, arising from, or in any way related to my (or my son/daughter/ward’s) participation in the Sports Activities. I also agree and undertake not to make any claim or take any proceedings against the Releases which might claim contribution or indemnity under the provisions of any statute or otherwise from any of the individual Releases.

5.  I UNDERSTAND that there are NO refunds for any reason unless my (or my son/daughter/ward’s) program is cancelled by VWH Consulting Inc.

6.  I UNDERSTAND that VWH Consulting Inc reserves the right to exclude, without refund, a player from participation in a program if the player’s health status generates a concern for the player’s, or other participants’ safety or well-being.

7.  I UNDERSTAND that all skaters must wear full hockey equipment, including helmet and bring a stick with that class sizes will be limited. 

8.  I UNDERSTAND that dressing rooms will be monitored by the parents, and VWH Consulting Inc or Vanessa Hettinger. VWH Consulting and Vanessa Hettinger shall have no responsibility or liability related to activities in the dressing rooms. 

9.  I UNDERSTAND that any participant’s names, photos, video are the properties of VWH Consulting Inc. and maybe used in future advertising and/or promotion for no fee.

10.  I UNDERSTAND that participating in programs requires full payment at time of registration, all payment schedules are to be approve by VWH Consulting Inc. Also, I understand that prices, instructors, dates, time and locations are subject to change of the program starting date.

1. I hereby grant VWH a non-exclusive right to use my images and likeness in photographs, audio and video recordings, or digital medias taken or to be taken on or between January 1, 2024 and December 31, 2024 the purpose of VWH’s internal and external business (“VWH’s Business”) (including but not limited to developing hockey and skating programs, coaching, mentoring, and training), in perpetuity, including but not limited to the right to edit, reproduce and distribute the photographs, audio and video recordings, or digital media without charge or requirement for my further permission or consent. I understand that the photographs, audio and video recordings, or digital media may appear in a wide variety of formats and be used in any and all possible and available publication formats and media, including without limitation websites and social media, television and related media, holograms, illustrations, print media and in other forms of publication.

2. I understand and agree that all photographs, audio and video recordings, or digital medias are the property of VWH. I acknowledge that I am not entitle to any compensation, royalties with respect to the use of my Images and likeness in photographs, audio recording and video recording, and digital media contemplated under this release.

3. I hereby waive any right that I may have to inspect and/or approve the finished product
or the copy that may be used in connection therewith, wherein my likeness appears, or
the use to which it may be applied.

4. I hereby release, discharge, and agree to indemnify and hold harmless VWH and its agents from all claims, demands, and causes of action that I have or may have by reason of this authorization or use of my photographic portraits, pictures, digital images, audio or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images or videotapes, or in processing tending towards the completion of the finished product, including publication on the internet and social media, in brochures, or any other advertisements or promotional materials.
