Greetings!  You should have at least one (1) nomination on the form. For each nomination, please write the nominee's full name and write at least one (1) paragraph explaining how the nominee demonstrates success in two or more of the criteria.  Click on the "Next" box to view the other categories to nominate an employee.
 Please note that you will be responsible for completing the nominator statement for the official packet if your nominee is selected as the school winner.
