Please confirm that you are in agreement with the following statements in relation to the installation of the chargepoint: 

• The chargepoint has been installed in an appropriate location, with appropriate access for a vehicle;

• The socket has a minimum supply of 3kw (not diminished by the simultaneous use of other sockets);

• Electrical equipment of different systems are at least 2.5 metres apart, for example a charging vehicle and another charging vehicle, or a charging vehicle and metalwork that is connected to a different earth system, such as a streetlight. This may mean two neighbouring houses are unable to both install cross-pavement solutions, or a cross-pavement solution cannot be installed near metalwork.  

• That the chargepoint being installed meets the Office for Zero Emission Vehicles (OZEV) Electric Vehicle Chargepoint Grant minimum technical standard specification Residential chargepoints: minimum technical specification - GOV.UK (www.gov.uk) and Electric vehicle chargepoint and infrastructure specifications - GOV.UK (www.gov.uk)

• That the installation is in full compliance with the requirements of the current edition of the UK wiring regulations (BS7671) and the IET Code of Practice for Electric Vehicle Charging Equipment Installation and that your installer can provide all evidence required of compliance with these documents, as specified by them.

• I have completed a BS7671 Electrical Installation Certificate and Building Regulations compliance certificate for this installation.

• I will notify the local Distribution Network Operator of this installation within 28 days, providing all information required on the form accurately.
