Each parent/guardian of a participant age 16 or 17 team must carefully read and sign a release, waiver and indemnity form in order for the youth to participate in Strikeout. Please sign the below form. 

In consideration of Toronto City Mission accepting this application, I, for myself, my heirs, executors, administration, successors and assigns hereby release, waive and forever discharge Toronto City Mission and all other associations, sanctioning bodies, sponsoring individuals and sponsoring companies and all their respective sponsors, agents, officials, servants, contractors, representatives, elected and appointed officials, successors and assigns of and from all claims, damages, costs, expenses, actions and causes of action, whether in law or equity in respect of death, injury, loss of damages to my person or property howsoever caused, arising or to arise by reason of my participation in the said events, whether as a spectator, participant, competitor or otherwise, whether prior to, during or subsequent to the event and notwithstanding that the same may have been contributed to or occasioned by the negligence of any of the aforesaid.

I further hereby undertake to hold and save harmless and agree to indemnify all the aforesaid from and against any and all liability incurred by any of them arising as a result of, or in any way connected with my participation in any of the said events by submitting this entry. I acknowledge having read, understood and agreed to the release, waiver and indemnity. I warrant that I am physically fit to participate in this event.

I also give permission to Toronto City Mission to use photographs and any other media representation of myself and my family members at the discretion of the mission. I release Toronto City Mission, its officers, directors, volunteers and the staff from any and all responsibility/liability that may arise as a result of the use of such photos/media.
