Carpool Tag Number Sign Up 2020-21

This form is to sign up for a carpool number that will be assigned to each family. At dismissal, the number will be used to inform students their designated family member has arrived on campus to pick them up. One tag number will be assigned per family then sent home with the student. Please display the number (dashboard or hang on the rearview mirror) so it is visible to West Lake staff during dismissal.
