ALLERGY AND LIABILITY WAIVER

I understand that I will be in direct contact with various cosmetics and products topically applied. I am aware of the specific chemicals and/or cosmetics to which I am allergic or have a sensitivity to.

I accept full personal responsibility for any personal injuries or allergies sustained by myself which might incur as a result of participating in this program. I discharge and hold harmless Breanna Cooke, LLC, their owners, directors, members, employees and agents from any claim, cause of action or liability for damages arising from any personal injury to me or other persons or property caused by my participation in Breanna Cooke, LLC’s workshops.

Any skin condition should be reported by the client prior to application and, if need be, a sample test of makeup may be performed on the skin to test reaction. The undersigned agrees to release Breanna Cooke, LLC from liability for any complications due to participation.


PHOTO RELEASE

I AGREE to give Breanna Cooke permission to use photographs, videos or audio recordings of myself for any Paint Your Date or workshop promotional materials. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.

I also understand that Breanna Cooke is not responsible for any expense or liability incurred as a result of my participation in this recording, including medical expenses due to any sickness or injury incurred as a result.

I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
