I, the parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the U.S.Y.S.A, its affiliated organizations, and sponsors. Recognizing the possibility of physical injury associated with Soccer and in consideration for the U.S.Y.S.A Accepting the registrant for its soccer programs and activities (the program), I hereby release, discharge and or otherwise indemnify the U.S.Y.S.A., its affiliated organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities utilized for the programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the programs and/or being transported to or from the same, which transportation I hereby authorize. Any behavior by the parents and/or their child deemed to be inappropriate and inconsistent with Great Kills Soccer Club and/ or Staten Island Youth Soccer League’s rules and regulations could result in the child’s removal from the program.

I agree and understand that by signing the Electronic Signature Acknowledgment below, that all electronic signatures are the legal equivalent of my manual/handwritten signature and I consent to be legally bound to this agreement.
