Please outline any experiences with dogs that they have had.   Please advise how you would manage introductions and interactions between the child(ren) and the dog *
Do you and/or the named primary carer above, or  anyone you will be relying on to care for the dog in your absence,  have any existing physical or mental health or mobility issues which may impact on your/their ability to meet the dog’s needs either now or in the future (please consider the age of the dog applied for and their likely lifespan when responding to this question)     If the answer is "yes" please give details of the alternative care you intend to put in place for the dog to ensure their needs will continue to be met if the named primary carer or anyone you will be relying on to care for the dog should be unable to do so   *
