Every member who wishes to partake in ELAPS activity at the Village Hall must complete this short form.

All data submitted will only be used for the purposes of administering the sessions and by responsible members / the committee of ELAPS, passed on to the relevant authorities as requires in the case of Track & Trace measures being necessary.
