Club Registration Form: Fall 2021
All clubs and interest groups must fill out the following information to be registered with Medical Student Government (MSG). The deadline is Friday, July 23, 2021, at midnight.

If you are applying to be a new club, you must have all of the information required on the first page prior to submitting your application.

If you are an existing club and would like to apply for MSG funding, please indicate so on page 2. You will be prompted for more information on the following pages. If you are not applying for funding from MSG, please indicate this and you will not have to enter information about your events.

For each event for which you are applying for funding, you will be prompted with a new page asking for: the name, type of event, description, month in which it takes place, location, estimated attendance, funding needs, educational purpose/benefit, and estimated cost. Please compile this information with your co-leaders to the best of your ability in order to fill out this form.

For SIM Events during COVID there are 30 suture pads available for check out through the SIM Center. There are no other SIM Events options available at time of Fall registration, will update accessibility when applicable. Any questions regarding SIM events, contact tannerwhiting@email.arizona.edu

If you are registering a club after the Friday, July 23, 2021 deadline, you must notify the VP by emailing vice-president@uacomps.org before your registration can be approved
