Join us on December 24th at 4pm for a short outdoor Christmas Eve worship service.  Please fill out both sections of this form to give us an idea of how many will be able to attend.

* On the day of the service, please bring a mask (extras will be on site if needed), a comfy chair, and a warm coat

* When you arrive, you will need to check-in at our greeter desk. They will sign you in and ask you to fill out the Contact Tracing Form if you did not complete online

* Family groups can sit together but please allow 6 feet spacing between groups as you are setting up your space
