Request for a Letter of Recommendation / Reference
To help Prof. Brianna Heggeseth provide the strongest recommendation or reference for you, please fill out this form. 

It is designed to help you do some self-reflection to improve your own application and to help her align her letter to your application. The more details you provide here, the stronger the letter she can write.

In addition to this form, please email Brianna the following:

a copy of your transcript (In 1600Grand, look for Academic Transcript and print to pdf the Web Transcript),
a copy of your personal statement/cover letter for your application (draft is fine),
a copy of your resume/CV (if you have one and if it is part of the application)

Once Brianna has agreed to write you a letter, please open all online applications as soon as possible (even if you aren't ready to submit them) and add her information to the application portal so that she is sent the information needed to submit the letter. 

She needs the instructions of how to submit the letters at least 2 weeks prior to the deadline and prefers to submit letters to all institutions at the same time.
