Club Session:  SESSION III
Club Sponsor:  Mrs. Grzybek and Ms. Kutrumanes

Grade(s): 5th - 6th
Meeting Day/Time:  Thursday, 3:00p.m. - 4:00p.m.

Club Overview:
Students will participate in group workouts.  Teachers will also share nutritious meal options and additional exercise videos.

A parent/guardian should complete this form.  The application serves as a permission form for your child to participate in the club.  

Depending on the number of students who apply, students may be randomly selected to participate.  All students will be contacted whether or not they are selected for a club.  By submitting this form, your child is committing to attend club sessions.  

SESSION III clubs are scheduled to begin the week of March 8.  Additional information, including a Zoom link and specific dates for this club, will be provided by the club sponsor once enrollment is closed.
