An interactive, outdoor program where families will learn about the great outdoors, make crafts, and enjoy a 30-minute hike around a different park each month!

COB Waiver & Release of Liability, Permission & Authorization Form: In consideration of being allowed to participate in the City of Burlington’s program listed above, and related events and activities, I hereby acknowledge and agree on behalf of myself and my minor child(ren) as follows:I RELEASE AND AGREE TO HOLD HARMLESS AND INDEMNIFY the City of Burlington, its elected officials, employees, agents, and volunteers, and its Recreation & Parks Department from and against any and all liability, and from and against any claims, demands, costs or expenses, or causes of action, of whatever kind or nature, which may arise from or relate in any way, directly or indirectly, with my and/or my child(ren)’s activities related to the City of Burlington, or my or my child(ren)’s presence at any location occupied or controlled by the City of Burlington, its employees or supervisors.I expressly acknowledge that I am aware that the above trip/activity/program, even under the safest of conditions possible, may involve risk to me or my child(ren). Nevertheless, I expressly assume all risk or hazard related or coincidental to, arising out of or connected to, the above trip/activity/program, including risk of loss or damage to personal property and/or personal injury, including permanent disability or death, however caused. I assume all the forgoing risks and accept personal responsibility for all expenses, medical or otherwise, following any such damages, injury, permanent disability or death to myself or my minor child(ren). Participating in City of Burlington Parks & Recreation programs or accessing City of Burlington Parks & Recreation facilities could increase the risk of contracting COVID-19. The City of Burlington and its Parks & Recreation Department in no way warrants that COVID-19 infection will not occur through participation in City of Burlington Parks & Recreation programs or when accessing City of Burlington Parks and Recreation facilities.I permit the City of Burlington to use and publish photographs and/or video of me and/or my children for purposes of promoting recreation activities to the community, news releases, or for other documentary purposes of the City. I give my permission to the City of Burlington its employees, its agents, and/or contractors and its volunteers the full authority to take whatever action they feel is warranted under the circumstances regarding my health and safety. ACPL Library Waiver and Agreement: This release is for any and all liability for personal injuries and property losses or damage occasioned by, or in connection with, Grow Wild! A Summer Hiking Series for Families. Read the following information very carefully and be sure that you understand it fully before agreeing. You MUST agree to this waiver before participating in the activities. The undersigned agrees to abide by all rules and regulations set forth by the Alamance County Public Libraries and/or its affiliate groups and vendors throughout the program or activities. I, the participant, have read the Alamance County Public Library’s policies and am fully aware that participating in the program activities may result in risk of personal injury or harm. I hereby release and hold harmless the Alamance County Public Libraries, its directors, officers, employers, volunteers, committees and boards, from and against all liability, and I and any parent or guardian signing for me below hereby agree to indemnify them against any loss, damages, claims, costs (including attorneys’ fees) or actions of any type, resulting from bodily injury, property damage, or other loss that may result from my participation in any program. I have read and understand this release, indemnification, and hold harmless form. I voluntarily agree to it and hereby give my permission to the Alamance County Public Libraries for emergency transportation and/or treatment in the event of illness or injury. I hereby accept responsibility for payment of any emergency transportation and/or treatment. I further certify that I am in good physical condition, and have no medical or physical conditions that would restrict my participation in this activity or program. In the event of a dispute arising out of this Waiver and Agreement, venue shall be proper and shall lie exclusively in the Superior Court of Alamance County. *
