After you fill out this form, we will contact you by email with more information about how we will arrange to collect your quilt (s). Please note that if you are planning on sending more than one quilt, we will need one form for each quilt in order to be able to get the correct number of spaces arranged in town and the correct information printed about each of your quilts.
The questions are a little different this year to accommodate quilts that may be entered by people who did not make them (especially antique quilts). We will be using the information first provided (name, address, email, etc.) to keep track of your quilts and contact you. The information provided in the second part of the form will be used in the quilt cards that are printed for the show. If you leave the second part blank, your quilt cards will be incomplete, so please fill everything out fully!
