CDP Second Life User Intake
While you may have provided some of this information during your initial event registration, we ask that you complete the form below to keep our attendee records up-to-date. Please complete this form for EACH CDP training you take in Second Life. Failure to provide complete and accurate information may result in your registration being cancelled, as you will be unable to access CDP's Virtual Education Center.

If you have not registered an account with Second Life, you can do so at the link below. Once you have registered your account, you MUST log into it to make ensure that the system can find you and grant you access.
