By signing this contract you give the academy permission to use photos of our students on the Academy Website, Instagram Account and Facebook Account. Please note that no parent is allowed to comment on the photos regarding locations and schools. Rather share to your own page to your own friends. This is to protect the students. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to: printed materials (eg - brochures and newsletters) online and offline advertising and promotion videos and digital images such for use on Social Media. By signing this form, I acknowledge that I am giving permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Etiquette Academy New Zealand for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images. *
I am allowing my child to participate in Etiquette Academy New Zealand activities. I hereby assume all the risks associated with the Academy. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Etiquette Academy New Zealand and it’s employees of any and all liability which may arise as a result of my child’s participation in activities at the academy *
Is there anything we should be aware of when teaching your child? This can be anything at all that you want to bring to our attention?.
Students will receive a complimentary photo shoot worth $200 and a Certificate upon completion.
Thank you for taking the time to fill out our registration form. A $15 donation per registration will be made to Cystic Fibrosis Waikato.
