Use this form to book your child into breakfast and after school clubs each half term. Please complete a
separate form for each child.
You will be billed in advance for the sessions that you book. Children recieving Pupil Premium will not be charged but must still be booked.
Bookings are taken on a first come, first served basis. The confirmation of your booking will be your bill for the half term. If the club is full, you will be informed that your child does not have a place.
