Ebenezer Middle School Chromebook Request Form 2022-2023
Students will need access to a Chromebook or laptop to complete assignments both at school and at home during the school year. Students are encouraged to provide their own personal Chromebook/laptop if possible. If this is not possible, one will be provided by EMS. This form is meant for parents/guardians of EMS students who will need a school-issued Chromebook/laptop for the 2022-2023 school year.  Students and parents/guardians will be responsible for lost or damaged school-issued technology (per the ECSD Student & Parent Handbook, pg 19). Please complete a separate form for each student.

I have read, understand, and agree to the following information as stated in the ECSD Student & Parent Handbook. "Technology issued to students is the property of the Effingham County School System. There is no charge for the use of technology such as Chromebooks and/or other technology items. If the Chromebook is lost, stolen, or damaged beyond repair, the cost of a replacement Chromebook is approximately $350. Additionally, repair costs for broken screens, keyboards, etc. are typically assessed at $50 per repair." *
