UEVA 2021 Clinic Registration Form - Boys

Please fill out this form completely to register for a 2021 UEVA clinic! If you are signing up for more than one clinic, please fill out this form once per event you wish to attend.

The clinic will have the following format and be maxed out at 40 athletes:

Check in: 8:30 am - 9:00 am
Session 1: 9:00 am - 12:00 pm
Lunch / Recruiting Q & A: 12:00 pm - 2:00 pm
Session 2: 2:00pm - 5:00 pm

** Please note that UEVA clinics are intended for athletes who are in middle and high school. **

Cost: AAU Members - $100
Non-AAU Members - $115, one time charge ($15 will go to cost of AAU membership, valid for 1 year)

** We will collect payment via Venmo at @UEVAdak prior to the event. Those wishing to pay the day of can do so in cash, check, or Venmo.**
*Checks must be made out to Dakota Pruiss.
