Please complete this form to request Short Term Independent Study.   If your child attends Da Vinci Charter Academy or elementary school, please contact their office for their procedures.

Short Term Independent study does not start until September 14, 2020.

Short Term Independent Study is approved for a minimum leave of 5 school days per occurrence.  Absences that extends beyond the approved dates may trigger a truancy letter since these additional days are not state approved absences. As long as the student does not incur additional unexcused absences over the course of the school year, the letter will not be followed by further action.

Please note that the last day that DSIS will accept travel study requests is May 14th.  All students must return from Short Term Independent Study by May 28, 2020 to allow teacher’s grading time before school ends.

If your child attends Davis Senior High School, Emerson Jr. High, Harper Jr. High, Holmes Jr. High or King High please fill out the request form.  If your child does not attend one of these schools, please contact your school to  request short term independent study.
