Class Transfer Request Form
On behalf of the School of Dance, we understand you wish to transfer your child into a different class. Any transfer request must be made by filling out this Class Transfer Request Form and submitted prior to Saturday, October 21. Notifying the instructor, staff member or other representative of Ballet Hispánico verbally, written or otherwise that a student would like to transfer from a class is not sufficient nor is it deemed an official notice of transferal. The transfer request will be processed in one week of receipt, and you will be notified via email once all has been confirmed.
