RTMF Children's Online Competitions Entry Form: Duet or Group Performance
Please fill in this form to register your duet/group's video entry to the Rothbury Traditional Music Festival competitions. Please see Rothburymusicfestival.co.uk for more information, including Guidelines for Entry, Parental Consent Form and a GDPR statement. Only fill in ONE of these Google forms for each group entry - but make sure a Parental Consent Form is filled in for EACH GROUP MEMBER. A signed Consent Form for each child MUST be received before your group's performance video will be forwarded to the judges for consideration.

We will return signed copies of the Parental Consent Form (downloadable from the festival website at www.rothburymusicfestival.com) for each child in the group *
Please note that submission of this Google form registers a video for entry in the competition but does not constitute full parental consent. Please download, print and sign the Parental Consent Form at www.rothburymusicfestival.co.uk and then return either digitally (scan or photograph) to rtmf.kidscomps@gmail.com or via post (address available on request). Please return a Consent Form for EACH CHILD IN THE GROUP.
