Before Training you will go through the Full COVID-19 Training Policies on the Club Website with the Player and ensure that they understand what they need to do and what not to do when training *
If you, the player, or anyone in the household exhibit any symptoms of COVID-19 you agree not to bring them to training sessions for at least 14-days and to take medical advice as appropriate and in line with Government Guidance. You will also notify the Club Secretary, Paul McMahon  on thedoc1959@yahoo.co.uk *
Finally, by selecting YES below you are agreeing in FULL to the COVID-19 Training Policies set in place by the Club. This also means that your details can be supplied to Public Health England as part of the NHS test and trace process, should anybody in your team's training sessions show symptoms or develop COVID-19.  NOTE: you can withdraw your consent, should you wish, at a later date by sending an email request to the Club Secretary, Paul McMahon on thedoc1959@yahoo.co.uk *
