NOTE: Families entering the Ontario School System for the first time and whose first language is a variety of English or another language, entering from a French Language school board, and returning Canadians after an absence of one year or more, should contact the HDSB Welcome Centre at welcomecentre@hdsb.ca.

DOCUMENTS:

- Completed Registration Form
- Proof of child’s age: birth certificate or passport or baptismal/faith record for your child
- Proof of address (any two of the following documents): lease or deed, car registration, utility bill, residential telephone bill, moving bill, property tax bill, bank statement, credit card statement, correspondence with a government agency. **Important Note: A driver’s license will not be accepted as documentation for “proof of address”.**
- Proof of child’s citizenship: birth certificate or passport, Record of Landing (IMM 1000)or Permanent Resident Card
- Proof of custody: if you are not the parent and your child is under 18 you must provide proof of custody (court order)
- Copy of IEPs and/or Psych Assessments (if applicable)
- GWSS Course Selection Sheet

Students transferring from another Ontario high school and who are Canadian Citizens or Permanent Residents, must include:

- Completed "Application to Transfer" form and have it signed by the Principal/Vice-Principal of the last school attended
- Transcripts and Credit Counselling Summary
