Code of Conduct Reports
The purpose of this form is to submit reports regarding violations of the Code of Conduct of the Kansas OER Summit. A report may be submitted by anyone with information regarding a Code of Conduct violation.

Reports submitted through this form will be sent to members of the OER Summit Team, specified in Section V.

If you provide your contact information, someone will be in touch to confirm your report. If you wish to submit your report anonymously, you may omit your name and contact information.
