2022-2023 School Bus Application
The following form is an application for your child to use the school bus for the 2022-23 school year. 
All parents/caregivers will need to complete this form for each of your children (if you have more than one child) in order to apply for this service. Please note that you will be informed IF your application has been successful or not. 
Bus fee is the same whether taking one-way or round-trip. Please check the school HP for detailed information. This service is available for pre - G8 students. 

Application will be for the whole semester or year and you will be billed and asked to pay at a later timing. Cancellation will only be accepted for the following month and the fee for the month will needed to be paid. The fee from the following month will be reimbursed. Please email the school office by email office@oyis.org  by the 20th to cancel the service from the following month.
