Share Your Success Stories
Informed Consent:

Has there ever been a time during your college experience where you felt overwhelmed, lonely, or gloomy, but eventually you were able to overcome it? Do you want to share how you overcame those challenges with other students and make your voice be heard? "Successful Stories" are not just for traditionally "good" students, everyone could have "successful" moments. We are interested in hearing your story to open the conversation about mental, physical, and emotional health as a CETA student.

We are developing a website to highlight the inspiring stories of UHart students, detailing the challenges they've faced and how they overcame them. Similar to MIT Flipping Failure website (https://flippingfailure.mit.edu/), you're invited to share your story in either text with selfie pictures, or in self-recorded videos. To compensate for your time and effort, you will be offered a $10 amazon gift code for text+selfies, or a $15 amazon gift code for video upon completion. Your participation is voluntary, and if you prefer not to share selfies or videos publicly, we respect your decision not to participate in this project.

Only the principal investigator (PI), Prof. Song Wang, has access to your response. Your information will remain confidential and will not be disclosed to any third party without your explicit consent. The PI will contact you soon after you complete the form for the proceeding steps to receive your gift code.
