In consideration of the opportunity for me (or my child/ward) to participate in the VARIOUS BAND FUNCTIONS AND PRODUCTIONS FOR THE 2020-2021 SCHOOL YEAR, I do hereby authorize Miami-Dade County Public Schools to use, in perpetuity, my likeness, voice, or performance which has been recorder on tape, film or other media. Such use shall include but not be limited to the following areas: (1) instruction, (2) general education purposes, (3) promotional material/recruitment/social media, or (4) entertainment. I also authorize use of any biographical facts about me as required. I understand fully the conditions set forth in this document and agree to the conditions of this release. *
