Students: Please submit one form for each book you need.  You will be notified of the status of your request by email, so please check your email regularly.   DO NOT RENT OR PURCHASE A BOOK UNTIL YOU RECEIVE AN EMAIL FROM ME.  We may have a book available for you to check out for the semester.  If we do not have one for you to check out, you will be given permission to purchase or rent the book or e-text and receive reimbursement UP TO $50.  

*Textbooks are not reimbursed for summer courses.
