Please complete this application to become or remain a member of the Mentor Headlands Community Center. Membership is valid from the date payment is received until 12/31/2023. Membership entitles you to reduced pricing on Community Center rentals, advance notice of events, and the opportunity to participate in meetings, special events and more.

$25/calendar year - HOUSEHOLD MEMBERSHIP: Resident or property owner within the Mentor Headlands - being an area bounded on the north by Lake Erie, on the east to Richmond Road, west to Links Road and south to Route 2.

$35/calendar year - NON-PROFIT/CHARITY MEMBERSHIP (NON-VOTING): Non-profit/charitable organization

$50/calendar year - NON-RESIDENT MEMBERSHIP (NON-VOTING): Non-resident of the Mentor Headlands area (defined above)
